About The Position

Sage Group HealthCare Staffing is seeking a Home Health RN for a travel/contract position in New Lenox, IL. This role involves performing all aspects of home health nursing for assigned patients in their homes, including some virtual/telehealth visits. The nurse will prescribe, coordinate, and deliver care through the nursing process, maintaining professional nursing standards. Responsibilities include supervising CNAs, coordinating care with other healthcare workers, providing comprehensive initial assessments, initiating and modifying Plans of Care, establishing discharge plans, completing OASIS data, and providing patient care according to regulations. The role also involves assessing and addressing the educational needs of patients and families, promoting a safe environment, and maintaining patient privacy and confidentiality. The nurse will use the Matrix EMR system and be provided with an iPad. The territory is within 15 miles of New Lenox, IL, with a weekly caseload of 24-26 patients. Orientation is 2 days, with the first day in the office and the second in the field.

Requirements

  • Registered Nurse experience: 2 years - Required
  • Home health nursing experience: 1 year (recent/current) - Required
  • ASN Degree or Diploma in Nursing - Required
  • RN License for the State of Illinois – Required
  • AHA BLS Certification - Required
  • Valid Driver License - Required
  • Proof of Auto Insurance - Required
  • Covid Vaccination Proof - Required

Nice To Haves

  • Will accept candidates with strong Med-Surg experience if Home health experience is limited or not recent
  • Matrix EMR charting experience - Highly preferred
  • OASIS experience - Highly preferred
  • Start of care experience - Highly preferred
  • Prior Travel Nursing experience – Preferred
  • BSN Degree in Nursing - Preferred

Responsibilities

  • Performs all aspects of home health nursing for assigned patients in the comfort of their homes.
  • Performs some virtual/telehealth visits.
  • Prescribes, coordinates and delivers care through the nursing process.
  • Maintains the standards of professional nursing.
  • Supervises CNA and coordinates care with other healthcare workers as assigned.
  • Provides comprehensive initial assessments to all clients and on subsequent visits, continually assesses Physical, Psychosocial, Environmental, and community resources available utilizing the principles of growth and development over the life span.
  • Initiates the Plan of Care, re-evaluates the need for care, and modifies it as ordered/needed.
  • Establishes discharge plans with patients, physicians, other disciplines, and caregivers.
  • Completes OASIS data.
  • Provides patient care using current Federal, State and Agency regulations.
  • Assesses the educational needs of patients, families, caregivers and implements appropriate teaching strategies.
  • Completes documentation according to regulatory guidelines.
  • Promotes a clean and safe environment of care, utilizing the SAFE error prevention habits.
  • Provides the highest standard of privacy and confidentiality in matters involving patients, coworkers, and the hospital by abiding by the Standards of Conduct.
  • Perform some Telehealth/Virtual visits in addition to home visits.
