Registered Nurse, Home Health - Senior Living Visits
About The Position
BAYADA Home Health Care is seeking a Registered Nurse for a Part Time or Per Diem opportunity in Northern Baltimore County, Maryland. This role focuses on servicing seniors within 1-2 communities, reducing commute times associated with traditional home health. The position requires flexibility with early mornings, evenings, or weekends, and specific availability during weekday business hours. Experience with HH/HCHB/Oasis is required for Per Diem roles. As a home care Registered Nurse, you will be a key part of a multidisciplinary team providing rehabilitative care and skilled nursing to clients, enabling them to receive medical care at home.
Requirements
- A current license as a Registered Nurse in Maryland.
- A minimum of one year of recent, verifiable clinical (medical/surgical) experience.
- Graduation from an accredited and approved nursing program, as indicated by school transcript or diploma.
- Ability to work independently and manage time effectively.
- Strong interpersonal skills.
- Solid computer skills; prior experience with electronic medical records (EMR) preferred.
- Ability to travel to cases as assigned.
- Per Diem requires current HH/HCHB/Oasis experience.
Nice To Haves
- Prior home care experience strongly preferred, but not required.
Responsibilities
- Follow a designated care plan in accordance with patient's needs
- Make home visits to clients in designated geographic territories
- Perform assigned duties, including administration of medication, wound care, treatments, and procedures
- Monitor clients' conditions; reporting changes to Clinical or Client Services Manager
- Follow up with, execute, and properly document doctors' orders
- Perform client assessments as necessary
- Case management and coordination
- Accurately document observations, interventions, and evaluations pertaining to client care management and services provided, utilizing a state-of-the-art touch pad tablet
