About The Position

This full-time Registered Nurse position is for a Home Health Nurse Case Manager, primarily serving the Westmont, Hinsdale, Willowbrook, Hodgkins, Berwyn, and Cicero areas. The role involves developing and maintaining therapeutic relationships with patients, ensuring quality and cost-effective care, educating patients and staff, applying the nursing process for problem-solving, and demonstrating leadership and professional development. Requirements

  • Current Illinois RN license
  • A valid driver’s license issued by the Division of Motor Vehicles
  • Graduate from an accredited school of nursing
  • Earn 10 continuing education credits annually
  • One year of medical surgical experience
  • Demonstrated proficiency for the requirements for RN role
  • Good communication and interpersonal skills
  • Good documentation skills
  • Computer skills
  • Good time management and organizational skills
  • Familiarity with patient care equipment
  • Current CPR

  • Demonstrates ability to critique own nursing practice and achieves required certifications or Bachelor’s Degree.
