About The Position

We are seeking an experienced Registered Nurse (RN) – Home Health / Hospice for a 13-week contract in St. Albans, Vermont. The RN will provide skilled home-based nursing care, serve as Case Manager for assigned patients, develop individualized plans of care, and coordinate services with patients, caregivers, physicians, and interdisciplinary healthcare teams.

Requirements

• Minimum 2 Years of Registered Nurse Experience Required; Home Health and/or Hospice Nursing Experience Required.
• Current, Active Registered Nurse License Required; Applicable Vermont Practice Requirements Must Be Met Before Start.
• Strong OASIS Documentation and Home Health Case Management Experience Required.
• Experience With IV Therapy, Wound VAC, Complex Wound Care, Patient Education, and Interdisciplinary Care Coordination Required.
• Ability to Work Sunday–Thursday and Participate in Required On-Call Rotation Required.

Responsibilities

• Perform comprehensive patient and caregiver assessments and develop individualized plans of care based on clinical needs and established home health standards.
• Complete accurate and timely OASIS documentation and maintain all required clinical, regulatory, and patient care records.
• Serve as Case Manager for assigned patients, manage caseloads and visit schedules, and coordinate care with physicians and interdisciplinary healthcare professionals.
• Provide skilled nursing interventions including IV therapy, wound VAC management, complex wound care, medication education, infection prevention, and other applicable home care procedures.
• Evaluate patient response to treatment, provide patient and caregiver education, coordinate safe discharges or transfers, and maintain accurate documentation in McKesson.
