About The Position

This full-time Registered Nurse position focuses on Home Health Case Management in the Olympia Fields area. The role involves developing and maintaining therapeutic relationships with patients throughout their healthcare journey, providing individualized care, and assessing patient and family readiness for care planning. The nurse will assure and improve quality care by mobilizing resources, adapting to evidence-based standards, and participating in quality improvement initiatives. Patient and staff education is a key component, as is the application of the nursing process for problem-solving, clinical decision-making, and care coordination. Leadership, collaboration, and professional development are also emphasized, requiring the nurse to be a respectful and effective team member committed to self-improvement and the development of others. The position requires a valid RN license, a driver's license, and a minimum of one year of medical-surgical experience. The role involves working Monday-Friday, 8:00 a.m. to 5:00 p.m., with a weekend rotation, shared after-hours call, and two holidays per year. A sign-on bonus of up to $10,000 is available for eligible candidates.

Requirements

• Graduate from an accredited school of nursing
• Earn 10 continuing education credits annually
• One year of medical surgical experience
• Has as demonstrated proficiency for the requirements for RN role
• Good communication and interpersonal skills
• Good documentation skills.
• Computer skills
• Good time management and organizational skills
• Familiarity with patient care equipment
• Current Illinois RN license
• Current CPR
• A valid driver’s license issued by the Division of Motor Vehicles.
