As part of our commitment to delivering exceptional care, this role is designed for someone who can manage a comprehensive plan of care for each patient, ensuring consistent and effective treatments in a home-based setting. Note that this is not a remote job; candidates should be prepared to travel within their assigned locale to provide patient care. As a Registered Nurse Home Health Case Manager with Physicians Choice, you will have the opportunity to collaborate with a team of healthcare professionals, working together to deliver and coordinate optimal patient outcomes. Your role is critical in assessing patient needs, planning and implementing clinical care tailored to individual patient conditions, and consistently monitoring and documenting progress. This position involves direct patient interaction in their homes, thus requiring high levels of professionalism, compassion, and adherence to healthcare standards.

Requirements

  • Valid Oregon RN License.
  • Current Driver's License and reliable transportation to commute to patient homes.
  • Proven experience as a Registered Nurse, preferably in a home health or case management setting.
  • Strong clinical skills in patient assessment and acute care.
  • Excellent communication and interpersonal skills to interact effectively with patients, families, and healthcare teams.
  • Ability to work independently and manage time effectively while covering multiple patients.
  • Valid CPR Certification.
  • Current automobile insurance covering travel to patient locations.
  • Demonstrated ability to follow detailed instruction and maintain patient confidentiality.
  • Physical stamina to perform job duties, including the ability to drive and to stand or walk for extensive periods.

Responsibilities

  • Drive to patient homes to perform regular visits as per the healthcare plan.
  • Manage a caseload of patients, assessing healthcare needs and individual risks.
  • Develop, implement, and evaluate individualized patient care plans in coordination with a multidisciplinary team.
  • Monitor patient health status and progress in a timely and consistent manner.
  • Communicate with physicians and other healthcare providers to update and modify patient care plans as necessary.
  • Provide direct patient care, including administering medication, wound care, and other personalized interventions.
  • Educate patients and their families on disease processes, treatment, and home health care management strategies.
  • Ensure compliance with federal, state, and local regulations and organizational policies relating to home health standards and patient confidentiality.
  • Document all patient interactions, treatments, and care plan adjustments accurately and comprehensively in the patient care record.
  • Respond to emergency situations with promptness and proficiency, applying appropriate interventions and protocols.
