About The Position
Navitas Healthcare, LLC is seeking an experienced and compassionate Registered Nurse (RN) for a Home Health Care contract assignment in York, Maine. This opportunity is ideal for a skilled nursing professional with prior home health experience and strong knowledge of OASIS assessments. The RN will provide high-quality patient-centered care in home settings while collaborating closely with interdisciplinary healthcare teams to support patient recovery, independence, and wellness.
Requirements
- Active Registered Nurse (RN) license in the State of Maine
- Current BLS/BCLS certification
- Minimum 1 year of recent Home Health RN experience required
- OASIS assessment experience required
- Excellent communication and patient care skills
- Ability to work independently in a home care setting
Responsibilities
- Deliver skilled nursing care to patients in home health environments
- Complete and document OASIS assessments accurately and efficiently
- Monitor patient conditions and report significant changes to the care team
- Administer medications and prescribed treatments
- Educate patients and families on care plans, medications, and health management
- Maintain timely and accurate clinical documentation and timekeeping records
