About The Position

The Registered Nurse plans, coordinates, and delivers quality home health and hospice care to patients in their homes. This role is responsible for assessing patient needs, developing individualized care plans, providing skilled nursing services, educating patients and families, and collaborating with physicians and the interdisciplinary care team. Requirements

  • Graduate of an accredited school of nursing.
  • Current Registered Nurse (RN) license in the state of practice.
  • Current CPR certification.
  • Two (2) years of nursing experience preferred; home health, public health, or hospice experience preferred.
  • Excellent assessment, communication, and clinical skills.
  • Ability to work independently and manage priorities effectively.
  • Valid driver's license, reliable transportation, and automobile insurance.
  • Ability to perform the physical requirements of patient care, including lifting, transferring, bending, standing, and walking for extended periods.

Responsibilities

  • Complete initial and ongoing patient assessments.
  • Develop and update individualized plans of care.
  • Provide skilled nursing, symptom management, and palliative care.
  • Administer medications and treatments as ordered.
  • Educate and support patients, families, and caregivers.
  • Coordinate care with physicians and interdisciplinary team members.
  • Maintain accurate clinical documentation.
  • Participate in on-call rotation and supervise hospice aides as assigned.
  • Assess patient needs and develop individualized care plans.
  • Provide skilled nursing services and ordered treatments.
  • Monitor patient progress and adjust care plans as needed.
  • Educate patients and caregivers on disease management and safety.
  • Collaborate with physicians and other healthcare professionals.
  • Complete timely clinical documentation and reporting.
  • Assist with discharge planning and care transitions.
  • Participate in on-call rotation and supervise assigned clinical support staff.
