About The Position

Mass General Brigham Home Care is seeking a part-time Registered Nurse to join their Weekend Team, providing care in the South Shore territory. This role offers the autonomy, variety, and fulfillment of home health care, allowing nurses to practice at the top of their license. The position involves leading and coordinating individualized patient care plans, delivering 1:1 skilled nursing care in patients' homes, collaborating with interdisciplinary clinicians, providing advanced clinical treatments and patient education, and helping to prevent hospital readmissions and improve long-term outcomes. The goal is to bring advanced clinical expertise, compassion, and innovation directly into patients’ homes to help them recover, regain independence, and manage complex conditions.

Requirements

  • Graduate of an approved School of Nursing
  • Current Massachusetts license as Registered Nurse
  • Minimum of one year previous nursing experience in acute care setting
  • Travel required within MGB Healthcare at Home geographic area
  • Ability to work with various computer software required
  • Strong interpersonal and customer service skills
  • Ability to work with a diverse population
  • Ability to communicate effectively in writing, verbally and electronically
  • Ability to work as a member of an interdisciplinary team of health care providers
  • Ability to work independently, be self-directed and adapt to unpredictable circumstances
  • Ability to organize and prioritize work, and adapt to changing situations

Nice To Haves

  • Prior home care experience preferred
  • Knowledge of hand-held patient care computer devices preferred

Responsibilities

  • Lead and coordinate individualized patient care plans
  • Deliver 1:1 skilled nursing care in the home
  • Collaborate with interdisciplinary clinicians
  • Provide advanced clinical treatments and patient education
  • Help prevent hospital readmissions and improve long-term outcomes
  • Oversee and coordinate the full plan of care
  • Complete starts of care, resumptions of care, evaluations, or revisits
  • Provide quality, clinical services to patients in various settings in the community
  • Perform admissions process and case management responsibilities
  • Utilize thorough and timely electronic documentation and interdisciplinary communications
  • Provide skilled nursing care in the form of assessment, teaching, treatment, documentation and care coordination
