Requirements

Active New York State Registered Nurse (RN) License (Required)
CPR/BLS Certification (Required)
Ability to work independently while maintaining communication with the clinical team
Excellent interpersonal and communication skills

Nice To Haves

Minimum 1–2 years of nursing experience preferred
Home Care, Geriatric, Hospice, or Related Clinical Experience Preferred
Strong clinical assessment and critical thinking skills
Experience with Electronic Health Records (EHR) preferred
Valid Driver's License and Reliable Transportation Preferred

Responsibilities

Conduct comprehensive patient assessments
Develop and implement individualized care plans
Administer medications, treatments, and nursing interventions
Monitor patient conditions and communicate significant changes
Provide skilled nursing services, including: Wound Care, IV Therapy, Catheter Care and Management
Educate patients and families on: Disease Management, Medication Compliance, Home Safety Practices
Maintain accurate and timely clinical documentation
Ensure compliance with all state and federal regulations
Provide compassionate care and emotional support to patients and families
