About The Position

On Lok PACE (Program of All-inclusive Care for the Elderly) is a non-profit organization dedicated to the care of elders. The PACE model is designed to address the challenges faced by seniors with chronic care needs and their families by providing services within the community. This includes assistance with transportation, medication management, coordination of medical care, social interaction, and support for remaining at home. PACE participants receive in-home care services and transportation to a PACE center for primary medical care, social and recreational activities, and other senior care services. The organization emphasizes a collaborative system and seeks individuals whose expertise, drive, and passion can contribute to their mission of improving lives and advancing healthcare.

Requirements

  • Graduate of an accredited college or university with either an associate or baccalaureate degree in nursing.
  • Current California Registered Nurse license.
  • Two years of acute care experience required (ICU, ER or Critical care) or homecare experience required.
  • One-year experience working with the elderly or frail population required.
  • Ability to work as part of a multidisciplinary team.
  • CA Driver's license and willingness to drive and travel.
  • CPR certificate.

Nice To Haves

  • A California Public Health Nurse certificate is desired.
  • Experience in public health nursing desired.

Responsibilities

  • Offers direct skilled care and supervision of care to participants receiving home care services.
  • Evaluates, implements, and develops a home care treatment plan for participants.
  • Monitors participant changes and adjusts treatment plans accordingly.
  • Provides skilled nursing care as needed.
  • Instructs and supervises geriatric aides and LVNs in carrying out the home care treatment plan.
  • Teaches and counsels participants and family members regarding participant's care and safety.
  • Participates actively in the interdisciplinary team by offering clinical expertise in team planning and problem-solving.
  • Communicates effectively with participants, family, and team members.
  • Performs other related duties as assigned.
