About The Position

Care Advantage Inc. is seeking a dynamic Registered Nurse (Home Care Supervisor) to join their Personal Care Team. This full-time role involves clinical leadership, patient advocacy, and team supervision within Staunton, VA, and surrounding areas. The position offers a Monday-Friday schedule with no weekends, on-call, or holidays, competitive wages, and mileage reimbursement.

Requirements

Active RN license in Virginia (compact/multi-state accepted)
Minimum of 1 year in a nursing role (LPN-to-RN pathway acceptable)
Associate's Degree in Nursing or higher
Strong communication and presentation skills
Ability to work collaboratively across teams
Reliable transportation for home visits
Comfortable with technology (computer/iPad documentation)

Responsibilities

Meet new patients, explain services, and complete start-of-care assessments.
Develop individualized plans of care addressing medical needs, personal preferences, and family involvement.
Track patient progress, adjust care plans, and report changes in condition to physicians and care teams.
Teach or supervise household members assisting with patient care.
Instruct Home Care Nurses, Aides, and Personal Care staff on care plan details, expectations, and best practices.
Review caregiver documentation, monitor reliability, and provide constructive feedback.
Conduct in-home supervisory visits while staff are delivering services.
Lead or oversee training sessions for new staff, including safety protocols, clinical standards, and company values.
Ensure adherence to OSHA, DMAS, CCC Plus Program, and state licensure regulations.
Prepare and coordinate clinical notes, progress reports, and compliance records.
Participate in interdisciplinary team meetings and quality reviews.
Establish and maintain relationships with referral sources, community partners, and healthcare providers.
Represent Care Advantage in the community, promoting their mission.
Assist with organizational growth during mergers and acquisitions.
