About The Position
The Home Care RN is responsible for delivering high-quality nursing care to patients in their homes. This role emphasizes clinical excellence, patient advocacy, and education to support individuals in managing their health conditions safely outside of a hospital setting.
Requirements
- Active RN license in the applicable state
- Completion of an accredited nursing education program
- Current Basic Life Support (BLS) certification
- Previous clinical experience required
- Strong critical thinking and decision-making skills
- Effective communication and interpersonal abilities
- Self-motivated with the ability to work independently

Responsibilities
- Perform initial and ongoing patient assessments to identify care needs
- Design and implement individualized nursing care plans
- Deliver direct patient care, including medication administration, wound care, and chronic disease management
- Observe and document patient status, reporting significant changes to the physician
- Provide guidance and education to patients and caregivers on health maintenance and recovery
- Coordinate services with interdisciplinary team members to ensure continuity of care
- Maintain accurate, compliant documentation in accordance with healthcare regulations
- Promote a safe and supportive home environment for patients
