About The Position

Care Advantage Inc. is seeking a dynamic and compassionate Registered Nurse (RN) Home Care Supervisor to provide clinical leadership and oversight throughout the Martinsville VA and surrounding areas. This role involves leading, mentoring, advocating, and making a meaningful impact in the lives of patients and families receiving care at home. Requirements

  • Active Registered Nurse (RN) license in VA (Compact License accepted where applicable)
  • Associate Degree in Nursing (ADN) or higher
  • Minimum of one (1) year of nursing experience
  • Strong leadership, communication, and interpersonal skills
  • Ability to work independently and collaboratively
  • Reliable transportation and willingness to travel throughout the service territory
  • Proficiency with electronic documentation and technology

Responsibilities

  • Conduct comprehensive patient assessments and admissions, introducing patients and families to available services and care expectations.
  • Develop and oversee personalized plans of care, collaborating with physicians, families, and caregivers to ensure goals are met and adjusting plans based on changing patient needs.
  • Monitor patient progress and clinical outcomes, communicating effectively with physicians and interdisciplinary team members to ensure continuity and quality of care.
  • Educate and support family members participating in patient care, promoting patient safety, confidence, and successful care at home.
  • Provide clinical guidance and support to caregivers, aides, and nursing staff, reinforcing best practices and company standards.
  • Review documentation and caregiver performance, providing feedback, coaching, and support to enhance quality outcomes.
  • Conduct supervisory visits to observe care delivery, ensuring patient satisfaction, compliance, and service excellence.
  • Support orientation and ongoing education initiatives, promoting continuous learning and professional development.
  • Ensure compliance with Maryland, District of Columbia, and applicable regulatory requirements, maintaining adherence to company policies and clinical standards.
  • Complete and maintain accurate patient records and nursing documentation, ensuring timely and compliant reporting.
  • Participate in quality initiatives and interdisciplinary team meetings to drive continuous improvement in patient outcomes and service delivery.
  • Build and maintain strong relationships with referral sources, healthcare professionals, and community partners.
  • Represent Capital City Nurses and Care Advantage professionally throughout the community.
  • Support business growth initiatives while maintaining exceptional standards of care.
