Registered Nurse - Home Care
About The Position
On Lok Lifeways PACE (Program of All-inclusive Care for the Elderly) is a non-profit organization dedicated to providing long-term senior care. The PACE model is designed to address the challenges faced by frail seniors with chronic care needs and their families by offering services within the community. This includes in-home care, transportation, primary medical care, social and recreational activities, and other senior care services. The organization emphasizes the importance of a collaborative system and seeks passionate individuals to join their team and improve lives while advancing healthcare.
Requirements
- Graduate of an accredited college or university with either an associate or baccalaureate degree in nursing.
- Possess a current California Registered Nurse license.
- One-year experience working with the elderly or frail population required.
- Ability to work as part of multidisciplinary team.
- Two years of acute care experience required (ICU, ER or Critical care) or homecare experience
- CA Driver's license and be WILLING to drive and travel.
- CPR certificate.
Nice To Haves
- A California Public Health Nurse certificate is desired.
- Experience in public health nursing desired.
Responsibilities
- Evaluates, implements and develops a home care treatment plan for participants.
- Monitors participant changes and adjusts treatment plan accordingly.
- Provides skilled nursing care as needed.
- Instructs and supervises geriatric aides and LVNs in carrying out the home care treatment plan.
- Teaches and counsels participants and family members regarding participant's care and safety.
- Participates actively in the interdisciplinary team by offering clinical expertise in team planning and problem solving.
- Communicates effectively with participants, family and team members.
- Other related duties as assigned.
