About The Position

BrightStar Care of Hudson & St. Croix Valley is seeking a compassionate Part-Time Registered Nurse (RN) Case Manager to provide skilled Home Care services and care coordination throughout Washington County, MN, and St. Croix County, WI. This position offers a schedule within the timeframe of Monday–Friday day shift schedule with no weekends, a company vehicle for work-related travel, competitive pay, and excellent benefits. In addition to RN case management, we also offer private duty nursing cases, providing opportunities to deliver one-on-one skilled nursing care to clients in their homes. As an RN Case Manager, you'll oversee client care from admission through ongoing services while providing skilled nursing visits in the home. You'll collaborate with physicians, caregivers, and families to ensure each client receives safe, personalized, high-quality care. This role is ideal for nurses who enjoy autonomy, relationship-based care, and helping clients remain safely in their homes.

Requirements

  • Active RN license in Minnesota and Wisconsin
  • Current CPR certification
  • One or more years of nursing experience
  • Current TB test (or chest X-ray if applicable)
  • Valid driver's license with a clean driving record
  • Strong assessment, organization, and communication skills
  • Experience using electronic medical records

  • Home health or home care experience preferred

Responsibilities

  • Complete initial and ongoing nursing assessments
  • Develop and update individualized care plans
  • Provide skilled nursing visits and treatments
  • Coordinate care with physicians, families, and caregivers
  • Perform medication reconciliation and patient education
  • Monitor changes in condition and communicate appropriately
  • Promote client safety through home assessments
  • Supervise and support caregivers as needed
  • Maintain accurate electronic documentation
  • Ensure compliance with state, federal, OSHA, and HIPAA regulations
  • Participate in an occasional on-call phone rotation
