About The Position

The Boston Medical Center Geriatrics Ambulatory Practice provides Primary Medical Care and Nursing Care Management to adults age 65 and older as well as Comprehensive Geriatrics Assessment and Memory Disorder Assessment as consultative services. This nursing position is a unique one requiring a variety of specialized nursing skills to effectively to manage the complex medical and psychosocial needs of older adults within an interdisciplinary team. The registered nurse, adheres to the rules and regulations of the Massachusetts Board of Registration in Nursing. Is primarily responsible for professional performance and direct patient care. The role encompasses nursing assessment, diagnosis, planning, intervention, evaluation, and coordination of care for patients and their families across the health care continuum. The registered nurse is accountable for clinical, educational, quality and fiscal patient care outcomes using the model of care designed by the nursing department and in accordance with established agency policies, procedures, protocols, guidelines and standards of practice.

Requirements

  • Nursing degree: Diploma, ASN or BSN (preferred), Ability to obtain BSN within 4 years
  • Licensed to practice professional nursing as a registered nurse in the Commonwealth of Massachusetts required
  • Current Basic Life Support (BLS) certification required
  • Fetal monitoring certification required
  • Ambulatory Clinic experience preferred
  • BSN and 5 years of clinical experience as an RN functioning in a model of care delivery system that included care management.
  • Specific geriatrics nursing knowledge and skills
  • Keen telephone triaging skills to evaluate episodic acute and chronic clinical issues with critical reasoning and astute clinical judgment to expedite appropriate care given the complexity of this high risk elder population
  • Broad knowledge of community resources
  • Strong organization skills with the ability to multitask
  • Expertise in problem solving and decision-making

Nice To Haves

  • Advanced Cardiac Life Support (ACLS) certification preferred.
  • Experience working with Geriatric population is preferred

Responsibilities

  • Consistently and effectively utilizes all steps of the nursing process in the provision of timely, safe, efficient and appropriate patient/centered care.
  • Analyzes the assessment data to determine appropriate nursing diagnoses.
  • Identifies expected outcomes individualized to the patient.
  • Develops a plan of care that prescribes interventions to attain expected outcomes.
  • Implements interventions identified in the nursing and medical plan of care.
  • Evaluates the patient’s progress toward the attainment of clinical and educational outcomes.
  • Systematically evaluates the quality, cost, and effectiveness of nursing practice.
  • Acts in a professional manner relative to practice standards, statutes, and regulations.
  • The nurse acquires and maintains current knowledge in nursing practice.
  • The nurse contributes to the professional development of peers, colleagues, and others.
  • The nurse’s decisions and actions on behalf of patients are determined in an ethical manner.
  • The nurse collaborates with client, significant others and health care providers in delivering client care.
