Utilizes the nursing process as the frame of reference for practice as a professional registered nurse and provides direct nursing care to patients and families. Triage all clinical calls routed via call center – contact patient, perform phone assessment and consult with MD and/or coordinate follow up as appropriate. Manage and triage all patient portal messages – consult with MD as appropriate. RN recalls – follow up calls to patients as requested by provider in effort to increase access for more urgent appointments but ensure that patients seen for acute issues (rashes, infections, medication issues etc.) are doing well or assess if additional follow up is needed. Medication refills – following color coded algorithm – for all red light meds assure office visits/labs are all up to date per protocol prior to queuing refill – obtain labs/schedule follow up as needed. Coordinate with pharmacy team for all medications requiring PA, including biologics and updating patients. Coordinate VNA services. Coordinate with Infusion pharmacy team arranging infusion services both on and off-site arranging for home infusion therapy as appropriate. Coordinate other collaborative specialty referrals (COP, GI, Rheum, ID, Endo). Contacting patients with biopsy, lab and culture results, education on diagnosis/treatment plan and follow up. Ongoing Management and updating of Digital/paper pathology log to ensure all skin cancers have either pending surgery date or have been treated. Injections – education on biologics, loading dose/maintenance dosing, side effects, provide patient with self-injection training and/or administering medication in clinic (Chesnut Hill/Needham only). Post-op follow up as needed – suture removals, wound checks, dressing changes as well provide ongoing wound care for chronic wounds, coordinate with provider as needed. Provides direct care to patients and makes necessary nursing judgments. Responsible for systematically assessing the health care needs of individuals or groups and for the formulation of a care plan, its implementation and evaluation. Demonstrates the ability for decision making by integrating theoretical knowledge with practical experience in caring for patients. Coordinates the care of patients and directs assistive personnel in order to provide safe, effective, efficient, equitable and timely, patient-centered care. Demonstrates awareness of environmental forces on health care of patients and their families. Demonstrates the ability to identify resources necessary to implement the plan of care. Works in a collegial and collaborative relationship with other health professionals to determine healthcare needs of patients and families. Develops relationships with patients and families that maintain and communicate trust and respect. Communicates effectively in the exchange of information. Demonstrate the ability to act as a patient advocate. Maintains annual mandatory education requirements, which include emergency skills and unit based competencies.
