About The Position
Urban Health Plan (UHP) is seeking a full-time Registered Nurse to join their St. Lawrence Community Health Center (Family Health) in the Bronx, NY. The RN will provide comprehensive nursing support and patient care within a family medicine practice. Requirements
- Bachelor's or Associates Degree in Nursing is required.
- NYS Registered Professional Nurse, required.
- Knowledge/training in electronic health records and Microsoft Office.
- Excellent communication skills, compassion, empathy, and team-driven attributes are essential.
Nice To Haves
- Bilingual English and Spanish, Bangla, or Bengali is strongly preferred.
Responsibilities
- Triage patients and assist patients with their needs based on acuity.
- Provide quality nursing care and medication administration.
- Administer immunizations/vaccines to specified patients.
- Monitor patient reactions and side-effects.
- Assist the care team with pre-visit planning for high-risk patients.
- Ensure implementation of plans of care.
- Act as a role model and source of nursing guidance to medical assistants and phlebotomists.
- Assist with prepping patients as needed and provide patient education.
- Assist providers during examinations and treatments.
- Ensure proper sterilization procedures using germicides, sterilizers, or autoclave.
- Ensure departmental adherence to clinical policies and procedures.
- Maintain health records by documenting patient contact and updating patient profile.
- Implement standing orders.
- Check equipment, logs, and medical kits.
- Assist with ordering medical supplies, medications, and vaccines.
