Responsibilities

Deliver safe and effective nursing care to patients across the continuum of care
Assess patient needs and develop individualized plans of care
Coordinate and supervise patient care activities
Collaborate with physicians, advanced practice providers, therapists, and other healthcare professionals
Educate patients and families regarding diagnoses, treatments, medications, and discharge planning
Monitor patient progress and communicate changes in condition to the healthcare team
Maintain accurate and timely clinical documentation
Promote patient safety, quality outcomes, and an exceptional patient experience
