About The Position

DaVita Kidney Care is expanding its services by bringing industry-leading clinical quality directly to patients in skilled nursing facilities (SNF) through a new team called DaVita Skilled Nursing Dialysis Services (DSD). This initiative aims to reduce the burden on patients who are transported for dialysis three times a week and to lower avoidable healthcare costs. DSD Services teammates will leverage DaVita's extensive resources to develop and grow this new service line. The ideal candidate will have experience in dialysis, thrive in a start-up environment, be passionate about improving patient lives, and possess a proven ability to 'Get Stuff Done' (GSD). Comfort working autonomously and collaborating with cross-functional teams is essential.

Requirements

  • Current Registered Nurse (RN) license in the state of practice.
  • Minimum of 12 months of RN experience.
  • Associates Degree in Nursing (A.D.N) from an accredited school of nursing required.
  • Basic computer skills and proficiency in MS Word and Outlook.

Nice To Haves

  • Previous Chronic Hemodialysis, or Acute Dialysis experience is preferred but not required.
  • Training/Teaching/Healthcare Education background.
  • Certified Nephrology Nurse (CNN) or Certified Dialysis Nurse (CDN).

Responsibilities

  • Take responsibility for the complete ongoing care of each patient to ensure their dialysis treatment stays on track.
  • Troubleshoot and make sound decisions for patients related to their dialysis treatment.
  • Occasionally partner alongside SNF staff to coordinate care.
  • Conduct impactful and meaningful education to patients and their families in the SNF setting.
  • Independently demonstrate best practice techniques and deliver optimal dialysis care.
  • Supervise Dialysis Technicians delivering tandem care to meet program capacity and efficiency goals.
  • Provide comprehensive patient care in an independent setting.
  • Assess, diagnose, and implement complete care plans, looking at the whole patient.
  • Support other local SNF dens as needed.
