Additionally, the role includes empowering patients and families by teaching home care strategies, fluid restrictions, and dietary changes to help them manage their health and avoid repeat hospital stays. The nurse will also drive team care by partnering with nephrologists, dietitians, and social workers to build and adjust personalized treatment plans that improve long-term patient health.
