About The Position

Under the direct supervision of the Clinical Manager (or designee), this role coordinates the total nursing care for End Stage Renal Disease (ESRD) patients on chronic dialysis by assessing, planning, implementing, and evaluating care in accordance with established hospital and departmental policies and procedures. The Registered Nurse provides Dialysis education to patients and families. This role provides leadership for the healthcare team by working cooperatively with other renal team personnel in maintaining a standard of professional nursing practice in the clinical setting with consideration of the aging process throughout the life span, including all developmental stages as well as cultural patterns. Requirements

  • Licensure as Registered Nurse in the State of Rhode Island by the Rhode Island Board of Nursing or licensure as a Registered Nurse in accordance with the Nurse Licensure Compact agreement of the National Council of State Boards of Nursing.
  • Baccalaureate in Nursing preferred.
  • Certified Nephrology Nursing Certification preferred.
  • Must have current licensure to practice as a Registered Nurse in the State of Rhode Island or in another state that participates in the Nurse Licensure Compact.
  • Demonstrates knowledge and skills necessary to provide care to patients throughout the life span, with consideration of aging processes, human development stages and cultural patterns in each step of the care process.
  • Preferred 12 months of nursing experience, and an additional 3 months of specialized dialysis experience providing clinical nursing care to dialysis patients, in either a chronic or acute setting.

Nice To Haves

  • Baccalaureate in Nursing preferred.
  • Certified Nephrology Nursing Certification preferred.

Responsibilities

  • Provides education and direct patient care for those utilizing Outpatient Dialysis services.
  • Assesses and identifies each patient’s needs utilizing advanced assessment skills.
  • Develops individualized plan of care, establishing interventions and patient outcomes in collaboration with physicians, other health team members, patients, and their significant others.
  • Implements the plan of care, according to proper procedure.
  • Clinical observation and equipment monitoring, administration of medications.
  • Communicates relevant information regarding patient needs or circumstances to appropriate person and/or healthcare team.
  • Administers medications according to policy and procedures.
  • Obtains orders lab work and follows appropriate nurse driven protocols.
  • Documents nursing actions and makes adjustments based on the evaluation.
  • Troubleshoots dialysis equipment when necessary.
  • Provides leadership by delegating, supervising, and participating in safe nursing care activities provided by support staff.
  • Evaluates effectiveness of patient care and identifies opportunities for improvement.
  • Assesses and provides appropriate teaching based on patient and family needs.
  • Participates in unit-based quality improvement and research activities.
  • Utilizes all available resources to provide quality, cost-effective patient care.
  • Meets and maintains clinical competencies measured by pre-determined credentialing standards, mandatory/safety education, and completion of Basic Life Support and ACLS/PALS as appropriate.
  • Identifies growth needs and participates in educational opportunities and professional organizations.
  • Serves as a role model/preceptor to new graduate nurses and less experienced personnel.
  • Assumes other responsibilities as delegated by Unit Leadership.
  • Home Modality: Occasional phone coverage off hours to provide patient resource.
  • Yearly home visit to assess environment and supplies.
  • Helps patients manage supply inventory span.
  • Provides in services when appropriate to outside providers.
