About The Position

The Home Infusion RN Clinician 2 is an experienced capable clinician focused on expanding knowledge and skills. Continuum Home Health and Home Infusion is the home care department of UVA Health. We are an academic home care agency, providing care in the community for over 30 years. The Home Infusion Nurse delivers specialized medications and educates patients in the home settings. This RN, consistently provides effective direct care to patients in their homes for an assigned caseload of patients; works as part of the interdisciplinary team; oversees the care planning and care coordination for a variety of complex patients. Manages care and implements treatment plans at a refined skill level in collaboration with patients and their families. This RN works directly with Pharmacists, Physicians and other members of the health care team and ensures compliance with homecare standards and regulatory requirements. Seeks as well as provides feedback for improved clinical practice. Assumes a beginning leadership role but seeks mentoring in this process. This job description integrates the ANA Nursing: Scope and Standards of Practice, and the ANA Code of Ethics for Nurses with Interpretive Statements, and the UVA Nursing Professional Practice Model.

Requirements

  • Graduate of an accredited nursing program required.
  • Bachelor of Science in Nursing required within 5 years of hire.
  • 1 year of Relevant Experience
  • Licensed to Practice as a Registered Nurse in the Commonwealth of Virginia required.
  • Valid Virginia Driver’s License required.
  • American Heart Association (AHA) Health Care Provider BLS certification required.

Nice To Haves

  • Previous home health, or home infusion experience is not required.

Responsibilities

  • Administer infusion therapies including, but not limited to, antibiotics, biologics, hydration, parenteral nutrition, and other specialty medications.
  • Provide patient and caregiver education on medication administration, infusion equipment, and infection prevention.
  • Assess and monitor vascular access devices including PICC lines, midlines, and central venous catheters.
  • Perform dressing changes, lab draws, IV line placement, and medication management as ordered.
  • Monitor patient response to therapy and communicate clinical updates to physicians and pharmacists.
  • Coordinate care with hospital discharge planners, outpatient providers, and pharmacy teams.
  • Document assessments, treatments, and patient education in the electronic medical record.
  • Participate in an on-call rotation to support urgent patient needs.
  • Reflects the influence of the nurse’s relationship with self, colleagues and patient/family on the patient experience.
  • Encompasses clinical assessment, planning, prioritizing, coordinating and implementation of care.
  • Demonstrate knowledge of and actively participate in shared governance.
  • Encompasses professional development through formal education, professional certification, internal and external learning opportunities and recognizes the value of external professional organizations. Supports onboarding of new team members and precepts as applicable.
  • Includes adherence to clinical documentation guidelines, comprehension of outcomes data, engagement in performance improvement activities and commitment to standard work.
  • Is demonstrated by application of technologies that support patient care, actively seeking to implement evidence based practice and new knowledge generated by nursing research.
