This role is for a Care Team registered nurse (RN) in a Primary Care Community Internal Medicine Outpatient practice. The RN is accountable for coordinating nursing care, including direct patient care, patient/family education, and transitions of care. This involves both in-person and non-visit care interactions, requiring strong communication and collaboration with providers and other Care Team members. The RN supports professional nursing practice across various settings and the continuum of care, functioning within the Mayo Clinic Nursing Professional Practice Model. Responsibilities include planning, implementing, evaluating, and communicating nursing care. The role also involves leadership through preceptor activities, informal/formal leadership, and quality improvement efforts. Delegation of patient care is based on skill level, experience, patient acuity, fiscal accountability, and resource availability. Excellent communication, mentoring, and teaching skills are essential, with potential participation in committees or projects. Some employees may be required to drive their personal vehicle. Primary Care services encompass healthcare for existing medical needs, preventative care, acute care, and chronic condition management, delivered through various modalities including telehealth and web-based care. The environment is fast-paced and team-oriented, fostering long-term patient relationships. This role is eligible for TN sponsorship and welcomes F1 OPT holders, but is not eligible for F-1 STEM OPT extension. Successful sponsorship requires state licensure and VisaScreen or equivalent certification. Internal applicants must attach their three most recent performance appraisals.
