About The Position

Aptive Resources is hiring twelve Registered Nurses to support the Patient Business Office / Community Care program at the Dayton VA Medical Center. This is a clinic-based, Monday-through-Friday RN role focused on care coordination rather than bedside care: you will review consults and referrals, assess Veterans' clinical needs, coordinate follow-up on labs, imaging and procedures, take clinical phone calls, and keep the record complete so Veterans get timely care inside and outside the VA. You will work under the professional direction of the VA Nurse Manager as part of an interdisciplinary team that includes VA providers, Advanced Medical Support Assistants and community providers.

Requirements

Graduate of a school of professional nursing accredited by ACEN or CCNE at the time of graduation and approved by the state accrediting agency
Current, unrestricted RN license from the Ohio Board of Nursing
Current BLS and CPR certification from an American Heart Association vendor; ACLS also required if assigned to a specialty clinic
At least 1 year of direct patient care experience within the last 3 years
Fluent in spoken and written English
Able to pass VA credentialing (VetPro) and a federal Tier 1 background investigation, and to obtain a VA PIV badge before the first shift
Not listed on the HHS OIG List of Excluded Individuals/Entities
Verifiable education and work history with any gaps explained

Nice To Haves

Prior VA experience, or experience with CPRS/VistA and the VA electronic health record
Background in ICU, med-surg, telemetry/cardiac monitoring or emergency nursing
Experience in care coordination, case management, utilization review, consult management or community/referral care
Ability to interpret cardiac rhythms and operate defibrillators, EKG machines and cardiac monitors
Health and immunization documentation (due within 5 days of award and before the first shift; tests current within the past year)
Negative TB test (TST or IGRA), or a negative chest X-ray if the test is positive
Proof of immunity to measles, mumps, rubella and varicella
One dose of Tdap
Annual influenza vaccine (or documented contraindication, with masking during flu season)
Hepatitis B vaccination series with post-series surface antibody result, and completion of bloodborne pathogen self-study training

Responsibilities

Provide nursing care coordination for an assigned panel of Veterans: contribute to the patient assessment, determine priorities, identify nursing measures and therapeutic objectives, and evaluate outcomes
Own the follow-up on labs, imaging and procedures ordered at clinic appointments, and handle RN phone consultation and clinical review for the service
Determine the appropriate course of action on consults and referrals and communicate in person or by phone with referring clinicians and community health care providers
Obtain patient and pertinent family history, assess the patient, plan nursing care, and reassess and revise the plan as needs change
Instruct patients and families on prescribed medications, therapies and upcoming procedures; assess understanding and document teaching
Verify transcription of provider orders, carry out prescribed orders, and review laboratory data and the electronic record
Administer medications and IV therapies within scope when assigned to a clinic setting; recognize emergencies and initiate appropriate measures, including life-saving measures in the absence of a physician
Document all care and communications in the VA electronic health record (CPRS/EHR) before the end of each shift, and select appropriate E&M and procedure codes as directed
Report patient safety events in the VA Patient Safety Reporting System within 24 hours and report any patient complaint to the COR or designee within 24 hours
Participate in discharge planning, daily team huddles, and required meetings: weekly All Call, weekly Nurse Call, and monthly Admin meeting
Refer patient and customer service issues to the Nurse Manager, Supervisory Physician or Admin Supervisor
Maintain professional appearance and decorum, apply VA customer service (ICARE) standards, and protect patient privacy at all times
