Requirements

  • Associate’s Degree in Nursing (or related field) required
  • 2+ years of home health or behavioral health experience as a Registered Nurse
  • At least 1 year of leadership or team support experience
  • Current state Registered Nurse license
  • Comfort using Workday, HomeCare HomeBase, and MS Office tools

Responsibilities

  • Create a positive, inclusive team environment where clinicians feel supported and motivated.
  • Lead and collaborate with an interdisciplinary care team to deliver coordinated behavioral health services in the home.
  • Serve as a trusted partner to clinicians, physicians, patients, and families — ensuring clear communication and shared goals.
  • Coach and support team members through regular feedback, problem-solving, and guidance.
  • Promote teamwork and accountability while ensuring high clinical standards and timely patient care.
  • Oversee plans of care with a focus on collaboration, continuity, and patient-centered outcomes.
