The Hospice Registered Nurse Case Manager will conduct comprehensive patient assessments, including vital signs and medical history review. This role involves traveling to patients' residences to conduct visits and developing individualized care plans in collaboration with multidisciplinary teams. The RNCM will coordinate hospice care by working with other team members and patient family members to ensure needs are met. Responsibilities include performing evaluations for hospice care, consulting with physicians regarding patient condition changes, educating patients and families on treatment plans and medication management, and promptly submitting clinical notes. The role also requires participation in IDG and case conferences, performing on-call duties, conducting initial and ongoing comprehensive patient assessments, developing and implementing care plans, supervising and evaluating hospice aides, and providing care plans to the Hospice Aide and LPN as part of the IDG team. Other duties as assigned.
