The Hospice Registered Nurse Case Manager will conduct comprehensive patient assessments, including vital signs and medical history review. This role involves traveling to patients' residences to conduct visits and developing individualized care plans in collaboration with multidisciplinary teams. The RNCM will coordinate hospice care by working with other team members and patient family members to ensure needs are met. Responsibilities include performing evaluations for hospice care, informational visits, and consulting with physicians regarding changes in patient condition. The role also involves educating patients and families on treatment plans and medication management, submitting clinical notes promptly, and participating in IDG and case conferences. On-call duties are also part of the role, along with conducting initial and ongoing comprehensive patient assessments and developing/implementing the plan of care. Supervision and evaluation of hospice aides are required, as is providing care plans to the Hospice Aide and LPN as part of the IDG team. Other duties as assigned.
