About The Position

St. Croix Hospice provides compassionate end-of-life care and supports patients and their families in pursuing dignity, comfort, and respect. The Registered Nurse Case Manager delivers individualized, holistic care to patients in their homes, collaborating with a care team to create personalized care plans, lead end-of-life conversations, and provide symptom management with empathy and clinical excellence. The role is based in Manhattan, Kansas, and is full time, with a stated schedule of 8:00 a.m. to 5:00 p.m. and on-call expectations.

Requirements

• Associate, bachelor's, or master's degree from an accredited nursing program and Registered Nurse (RN) qualification.
• At least 1 year of recent acute care experience.
• Reliable transportation, a valid driver's license, and proof of auto insurance.

Nice To Haves

• Hospice or home health experience.

Responsibilities

• Complete initial and ongoing comprehensive assessments to determine hospice needs.
• Develop individualized care plans with patients and families and update them as needs evolve.
• Administer prescribed medications and treatments.
• Educate patients and caregivers on care techniques, medications, diet, and symptom management.
• Rotate on-call duties and ensure seamless care transitions.
• Collaborate with the care team to provide personalized support and exceptional symptom management.
• Lead meaningful end-of-life conversations.
