About The Position

The RN Case Manager is responsible for facilitating patient care along a continuum, ensuring optimal health outcomes, access to care, and appropriate resource utilization, while respecting patient resources and self-determination. This role involves ensuring care is provided at the medically necessary level, assessing transition needs for timely throughput, safe discharge, and preventing avoidable readmissions. The position integrates national standards for case management, including care coordination to drive progression of care, appropriate resource use, and patient safety through readmission prevention, all while considering patient choice and goals of care. It also involves educating physicians, patients, families, and caregivers, and ensuring compliance with state and federal regulatory requirements, TJC accreditation standards, and Tenet policy. Palm Beach Gardens Medical Center, part of the Palm Beach Health Network, is a 199-bed acute care hospital that has served the Palm Beach County and Treasure Coast communities for over 50 years. It was the first in Palm Beach County to perform open-heart surgery and is a leader in cardiac care, also offering award-winning stroke care, neurosurgery, urology, orthopedics, robot-assisted and minimally invasive surgical techniques, bloodless medicine, and a 24-hour emergency room.

Responsibilities

  • Facilitate care along a continuum through effective resource coordination to help patients achieve optimal health, access to care and appropriate utilization of resources, balanced with the patient’s resources and right to self-determination
  • Ensure that care is provided at the appropriate level of care based on medical necessity and to assess the patient for transition needs to promote timely throughput, safe discharge and prevent avoidable readmissions
  • Drive progression of care, appropriate use of resources, and patient safety through readmission prevention while simultaneously considering patient choice and goals of care
  • Provide education to physicians, patients, families, and caregivers
  • Ensure compliance with state and federal regulatory requirements, TJC accreditation standards and Tenet policy
