The RN Case Manager is responsible for facilitating patient care along a continuum, ensuring optimal health outcomes, access to care, and appropriate resource utilization, while respecting patient resources and self-determination. This role involves ensuring care is provided at the medically necessary level, assessing transition needs for timely throughput, safe discharge, and preventing avoidable readmissions. The position integrates national standards for case management, including care coordination to drive progression of care, appropriate resource use, and patient safety through readmission prevention, all while considering patient choice and goals of care. It also involves educating physicians, patients, families, and caregivers, and ensuring compliance with state and federal regulatory requirements, TJC accreditation standards, and Tenet policy.
