About The Position

Hospice Home Care is seeking a dedicated Hospice Registered Nurse Case Manager to provide expert, patient-centered hospice care in Monticello, Arkansas. The role is scheduled Monday through Friday, 8 a.m. to 5 p.m., and involves one-to-one care for patients and support for their families. The RN works independently while receiving team support and may participate in an on-call rotation as required by the local branch.

Hospice Home Care focuses on managing patients’ pain and other symptoms while providing emotional and spiritual support to families. Its interdisciplinary team provides holistic hospice services, including routine home care, respite, general inpatient care, and continuous care. The organization considers quality of life as important as length of life.

Requirements

• Graduate of an accredited nursing school.
• Current licensure to practice in the state of operation.
• At least one year of nursing experience.
• Licensed driver with an insured automobile in good working order, meeting state or organizational requirements.
• Commitment to clinical and documentation excellence.

Nice To Haves

• Three or more years of nursing experience preferred.
• Experience in hospice or a similar setting preferred.
• Knowledge of the hospice philosophy of care.

Responsibilities

• Assess and monitor patients’ physical, emotional, and psychological needs.
• Create hospice care plans aligned with each patient’s wishes and goals.
• Direct nursing care, including administering medications, treatments, and interventions.
• Provide pain and symptom management.
• Educate and support patients’ families and caregivers.
• Collaborate with the interdisciplinary team.
• Maintain accurate and timely documentation.
• Participate in the on-call rotation as required by the local branch.
