About The Position
Navitas Healthcare, LLC is seeking a Registered Nurse (RN) – Case Manager for an exciting Travel or Local job in Anchorage, AK. This position involves coordinating and managing patient care plans across the continuum of care, performing patient assessments for care level and discharge planning needs, and collaborating with interdisciplinary teams to optimize patient outcomes. The role also includes facilitating discharge planning and post-acute care coordination, monitoring patient progress, ensuring compliance with guidelines, documenting and tracking care using Epic EMR, advocating for appropriate utilization of healthcare services, communicating with patients and families, and supporting quality improvement initiatives.
Requirements
- Active Alaska RN license (required)
- Minimum 1 year acute care RN experience
- Epic EMR experience required
- BLS certification required
- Strong clinical assessment and care coordination skills
- Ability to work in fast-paced hospital environment
