About The Position
Coordinate the interdisciplinary plan of care from admission through discharge, ensuring seamless transitions and consistent, high-quality patient care. Monitor patient progress through ongoing assessments and medical record review to ensure care plans remain appropriate and goal-focused. Advocate for patients by securing necessary resources and removing barriers that may delay treatment, discharge, or recovery. Collaborate with physicians, nurses, and interdisciplinary team members to ensure timely implementation of the health plan and continuity of care.
Requirements
- Licensed Registered Nurse credentialed from the Texas Board of Nursing required.
- Diploma from an accredited school/college of nursing OR professional licensure at time of hire required.
Nice To Haves
- Previous critical care experience preferred.
- Previous charge or supervisory experience preferred.
