The Hospice Registered Nurse Case Manager will conduct comprehensive patient assessments, including vital signs and medical history review. This role involves traveling to patients' residences to conduct visits and develop individualized care plans in collaboration with multidisciplinary teams. The RNCM will coordinate hospice care by working with other team members and the patient's family members to ensure needs are met. Responsibilities include performing evaluations for hospice care, informational visits, and consulting with physicians regarding changes in patient condition. The role also involves educating patients and families on treatment plans, medication management, and health maintenance strategies, as well as submitting clinical notes promptly. The RNCM will participate in IDG and case conferences, perform on-call duties, conduct initial and ongoing comprehensive assessments, and develop/implement the plan of care. Additionally, the RNCM will supervise and evaluate the care given by the Hospice Aide and provide the care plan to the Hospice Aide and LPN as part of the IDG team. Other duties as assigned.
