About The Position

This role is for a Registered Nurse Case Manager in the Case Management Department at Via Christi, St. Francis in Wichita, KS. The position is full-time, day shift. The role involves creating comprehensive care plans, serving as a patient advocate, coordinating between hospital and post-acute services, guiding medical record documentation, and advocating for efficient resource utilization.

Requirements

Licensed Registered Nurse credentialed from the Kansas Board of Nursing obtained prior to hire date or job transfer date required.
Experience in Case Management, Discharge Planning, or Utilization Review.
Inpatient clinical experience.
Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire.

Nice To Haves

Case Manager credentialed from the American Case Management Association (ACMA) preferred.
Case Manager credentialed from the Commission for Case Manager Certification (CCMC) preferred.

Use your expertise to guide medical record documentation and provide level-of-care recommendations that align treatment options with the best possible outcomes.
Advocate for the efficient and effective use of clinical resources, balancing high-quality care with smart operational stewardship.
