About The Position

Texas Health Heart & Vascular Hospital is an independently licensed hospital located on the campus of Texas Health Arlington Memorial Hospital. It is a 47-bed facility with a 20-bed Cardiovascular ICU and a 27-bed cardiac telemetry unit. The hospital is accredited for Chest Pain through The Joint Commission and has received AHA’s Gold Plus award for three consecutive years, as well as the AHA’s Gold Plus award for Heart Failure. The hospital specializes in CABG and Valve surgeries, thoracotomies, general cardiac care including heart failure, cardiomyopathies, AMI, TAVR, Impella, and heart catheterizations (angioplasty, stenting, electrophysiology studies). Requirements

  • Associate degree Nursing Required
  • 3 years in an acute care setting, case management or utilization review Required
  • RN - Registered Nurse Upon Hire Required

Nice To Haves

  • Bachelor’s degree Preferred
  • One year of hospital case management experience is preferred.
  • BCCMN - Board Certified Nursing Case Management CPUR, CPHQ, CCS, PCUM Upon Hire Preferred

Responsibilities

  • Identify at-risk populations and patients at risk for re-admission, specifically those who have readmitted or are at high risk for readmission within 30 days, with an emphasis on HF, COPD, AMI, and Pneumonia patients.
  • Participate in the development of individualized discharge plans to meet patient needs and referral source requirements.
  • Establish and support realistic treatment/discharge goals with patients, families, and staff.
  • Apply nursing/clinical knowledge, skills, and problem-solving techniques to the case management process.
  • Manage aspects of discharge planning that require outside services for identified high-risk populations.
  • Maintain active communication with patients, family members, physicians, nursing staff, and other multi-disciplinary team members to develop treatment/discharge plans.
  • Coordinate specific focused discharge interventions and services.
