About The Position

The Registered Nurse - Case Manager will assess, evaluate, and drive the discharge plan for patients. This role involves coordinating services to ensure patients are moved to the appropriate level of care. The nurse will ensure that the treatment plan is initiated, evaluated, and re-evaluated daily, monitoring patient progress and identifying barriers to discharge. They will also monitor length of stay and adhere to Medicare regulatory guidelines regarding patient choice, IMM notices, and MOONS.

Requirements

• Minimum 3 years of experience.
• Unit does not accept first-time travelers.
• Full compliance required to start on time.
• Locals accepted.

Nice To Haves

• Candidate will be reviewed by clinical to determine clinical fit.
• If clinical pass and within Piedmont guidelines, candidate will be sent to the manager for final decision.
• Clinical screens will be sent on case-by-case basis.
