About The Position
This is an opportunity for a dedicated Registered Nurse Case Manager to join our client and make a difference in the lives of patients in hospice care. You will be part of a supportive team, providing essential care and coordination for patients and their families.
Requirements
- High School/GED
- Hospice (1+ years)
- Management (1+ years)
- 1+ years of experience in ANY of the following: Home Health Care Epic Home Health Home Health
- Valid Registered Nurse (RN) certification/license in the state of Illinois (IL)
- ALL of the following valid licenses/certifications: American Heart Association Basic Life Support (AHA BLS) American Heart Association Advanced Cardiac Life Support (AHA ACLS)
Responsibilities
- Manage patient care in hospice settings
- Coordinate with healthcare teams
- Provide leadership and support to nursing staff
Benefits
- Additional stipends and compensation for fulfilling on call
- (2) weekdays of 5P to 8A
- (2) holidays per year – 1 summer & 1 winter; 3 floating holidays
