About The Position

The Registered Nurse - Case Manager position is in the Case Management Department. This role is responsible for coordinating the overall interdisciplinary plan of care for patients from admission to discharge. The Case Manager creates a plan for care across the continuum, integrating patient/family preferences and values, while monitoring medical necessity and level of care. They advocate for resources, remove barriers, and maintain communication with supervisors and care transition team members. This role also serves as a resource for medical record documentation, level of care recommendations, and services related to diagnoses and post-discharge care treatment options.

Requirements

BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.
Currently licensed or eligible to be licensed as a Registered Nurse (RN) by the Alabama Board of Nursing required.
Diploma from an accredited school/college of nursing and professional licensure at time of hire required.

Responsibilities

Coordinates the overall interdisciplinary plan of care for patient, from admission to discharge.
Creates plan for care across the continuum, integrating patient/family preferences and values.
Monitors patient medical necessity and level of care through assessments, ongoing evaluations and/or patient records.
Advocates for resources and removal of barriers.
Maintains ongoing dialog with supervisor and care transition team members to ensure effective implementation and reevaluation of health plan.
Acts as a resource for adequate medical record documentation, level of care recommendations, and services as they relate to diagnoses, and treatment options for post-discharge care.
