About The Position

St. Croix Hospice is seeking a Registered Nurse Case Manager to provide individualized, holistic support to patients in their homes. The role collaborates with the care team to develop personalized plans of care, leads end-of-life conversations, and provides empathetic, clinically excellent symptom management. The position offers meaningful patient connections and autonomy in practice.

The role is based in Wildwood, Missouri, is full time, and has an 8:00 a.m. to 5:00 p.m. schedule with on-call expectations. Travel is listed as 100%.

Requirements

• Associate, bachelor's, or master's degree from an accredited nursing program (RN).
• At least 1 year of recent acute care experience.
• Reliable transportation, a valid driver's license, and proof of auto insurance.

Nice To Haves

• Hospice or home health experience preferred.

Responsibilities

• Complete initial and ongoing comprehensive assessments to determine hospice needs.
• Develop individualized care plans with patients and families, updating them as needs evolve.
• Administer medications and treatments as prescribed.
• Educate patients and caregivers on care techniques, medications, diet, and symptom management.
• Rotate on-call duties and ensure seamless care transitions.
• Collaborate with the care team to provide patient care and support.
