This full-time Registered Nurse Case Manager position is responsible for managing caseloads of patients in specialty areas. The role involves conducting initial assessments, establishing plans of care in collaboration with physicians and based on patient needs, and providing direct patient care. The nurse will adjust nursing care processes, identify problems, modify care plans, and evaluate outcomes. Responsibilities also include advising and consulting with families and other personnel, ongoing assessment of patient response to treatments, updating care plans, and completing accurate and prompt documentation (including visit assessments, OASIS, 485, and verbal orders). Communication with physicians to confirm and update care plans, providing status updates, and ensuring ongoing communication with the healthcare team are crucial. The role also facilitates discharge planning according to standards, utilizes resources for continuity of care, and participates in developing discharge plans with patients/families and the healthcare team. Quality improvement activities, including chart audits and process improvement initiatives, are also part of the role. Maintaining accurate clinical and patient records according to agency, state, and federal guidelines is required. The nurse must demonstrate knowledge and skills for age-appropriate care, understanding principles of growth and development, and assessing/interpreting patient data based on age-specific needs.
