About The Position

Mobile Health Team Inc. is seeking an experienced Registered Nurse - Case Manager Primary Care/Clinic Nurse for a 13-week travel assignment in Juneau, AK. This role involves participating in triage, responding to emergencies, and actively managing assigned patients. The nurse will serve as a clinic nurse, collaborating with the medical provider and interdisciplinary team to establish individualized medical goals and maximize patient healthcare outcomes. Responsibilities include coordinating care conferences for high-risk patients, managing acute and chronic care needs, and providing patient outreach and education. The nurse will also advocate for patients and families, facilitate referrals, track progress, and serve as a link for care coordination. Additionally, the role involves performing staff nursing duties such as assessment, documentation, education, and medication administration, while adhering to safety and infection control policies.

Requirements

  • At least 1 year of case management experience within the last 2 years.
  • Active Alaska license at time of submittal.
  • Graduate of an accredited school of nursing.
  • Current full, unrestricted Registered Nursing license in Alaska required.
  • High school diploma or equivalent required.
  • Must have and maintain current Basic Life Support (BLS) certification by the American Heart Association during employment.
  • Excellent communication skills.
  • Ability to work independently.
  • Strong organizational skills and ability to multitask.
  • Willingness to participate in continuing education and training programs.
  • Strong attention to detail.
  • Strong communication, professionalism, and reliability.
  • Proof of flu vaccine required.
  • Drug screen and background check (facility required).
  • Additional facility onboarding forms as assigned.

  • National Case Management certification preferred.

Responsibilities

  • Participate in the triage role and respond to emergency situations.
  • Actively manage patients as assigned and serve as clinic nurse as assigned.
  • Collaborate with the medical provider and interdisciplinary team to establish individualized medical goals.
  • Coordinate multi-disciplinary care conferences for high risk or complex patients.
  • Manage panel acute and chronic care needs and health maintenance through EHR reminders and patient outreach.
  • Participate in case management reviews with a multi-disciplinary team.
  • Act as an advocate for the patient/client family, fostering decision-making, independence, and self-care.
  • Use problem-solving and anticipatory skills to meet patient and provider needs.
  • Address patient needs in a prompt and efficient manner.
  • Facilitate the coordination of patient referrals within and outside facilities.
  • Track the progress of referrals and outcomes of care, providing status reports to providers.
  • Serve as the primary link for other case managers and discharge coordinators in care coordination.
  • Ensure referral records are received and available for provider review.
  • Proactively identify and remedy delays and obstacles to care.
  • Perform staff nursing duties including assessment, documentation, education, and administering medications/therapies.
  • Maintain annual competencies, including safety and infection control requirements.
  • Perform duties in a responsible, respectful, and caring manner.
  • Maintain a clean and safe working environment, reporting defective equipment and safety hazards.
  • Adhere to infection control policy and procedures.
  • Work collaboratively with team members within and across departments.
  • Participate in department and clinic team and improvement efforts.
  • Be an active member of the clinical team and a leader amongst the pod, nursing staff, and unit coordinators.
  • Participate in the continued development of the case management role in primary care.
  • Perform other duties as assigned, including special projects.
