This role involves working in either Home Health or Hospice, collaborating with a multidisciplinary team to create and implement personalized treatment and discharge plans. The nurse will coordinate services aligning with patient needs, payer guidelines, and clinical goals to ensure seamless care transitions.
Requirements
- Registered Nurse License Required
- At least one year of case management experience working in a clinical setting
Responsibilities
