About The Position
Community Home Health Care is growing and looking for passionate case managers to join their team. There is an immediate opening for a Bilingual Case Manager (English/Burmese).
Requirements
- Excellent communication & relationship-building skills
- Strong organization and time-management abilities
- Knowledge of Medicaid, Medicare, and Indiana home care guidelines
- Background in case management, social work, or healthcare
- Tech-savvy with EHR or case management systems
Nice To Haves
- Bilingual (English/Burmese)
Responsibilities
- Evaluate needs and design personalized care plans
- Partner with nurses, caregivers, and providers for seamless support
- Be a trusted voice for patients and families
- Keep records accurate and services aligned with state and federal guidelines
- Step in quickly when urgent issues arise
- Track progress and adapt care plans to improve outcomes
