About The Position

Ascension Sacred Heart Panama City is seeking a Registered Nurse Case Manager for their Case Management department. This is a part-time, weekend position. Requirements

  • Licensed Registered Nurse credentialed from the Florida Board of Nursing obtained prior to hire date or job transfer date required.
  • Licensed as a Registered Nurse in Florida required.
  • Credentialed from State Board of Nursing or current home state license for multi-state license recognition "Compact State" obtained prior to hire date or job transfer date required.
  • Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire.

Nice To Haves

  • Previous case management experience
  • BLS Provider preferred. American Heart Association or American Red Cross accepted.

Responsibilities

  • Coordinate the comprehensive interdisciplinary plan of care from admission to discharge, integrating clinical pathways with the unique preferences, cultural values, and goals of the patient and their family.
  • Continuously monitor patient progress through direct assessments and medical record reviews to evaluate the effectiveness of the health plan and ensure the appropriateness of services relative to the diagnosis.
  • Proactively identify and eliminate social, financial, or administrative barriers to recovery by advocating for necessary medical resources and facilitating access to post-discharge care options.
  • Maintain active dialogue with healthcare providers and supervisors to ensure effective plan implementation while serving as a resource for high-quality medical record documentation and regulatory compliance.
