About The Position

The Registered Nurse - Case Manager coordinates the interdisciplinary plan of care for patients from admission through discharge. The role develops care plans across the continuum while incorporating patient and family preferences and values. The nurse assesses and monitors medical necessity and level of care through evaluations and patient records, advocates for appropriate resources and removal of barriers, and maintains ongoing communication with the supervisor and care transition team to support effective implementation and reevaluation of the health plan. The position also provides guidance on medical record documentation, level-of-care recommendations, and services related to diagnoses and post-discharge treatment options. The position is in Case Management and is based in Birmingham, Alabama. The schedule is Monday through Friday, 8:00 a.m. to 4:30 p.m., with holiday rotations and a Saturday rotation every eight weeks.

Requirements

Currently licensed or eligible to be licensed as a Registered Nurse (RN) by the Alabama Board of Nursing.
Diploma from an accredited school or college of nursing and professional licensure at the time of hire.
BLS Provider certification obtained within 30 days of hire or job transfer; American Heart Association or American Red Cross certification accepted.
