About The Position

Home Health Registered Nurse (RN) Part Time and Full Time available in Rapid City. At Interim HealthCare of the Black Hills, we're looking for a compassionate and motivated RN Case Manager to join our Home Health team. As an RN Case Manager, you'll have the opportunity to care for patients throughout their healthcare journey—developing personalized care plans, coordinating with an interdisciplinary team, and helping patients safely recover where they feel most comfortable: at home. Requirements

  • A licensed Registered Nurse (RN) in South Dakota.
  • Passionate about delivering patient-centered care.
  • Organized, self-motivated, and able to work independently.
  • An excellent communicator with strong critical thinking skills.
  • Comfortable using electronic medical records and completing clinical documentation.
  • Dedicated to teamwork, collaboration, and continuous learning.

Nice To Haves

  • Home Health experience is preferred but not required.
  • We welcome experienced nurses from hospital, clinic, long-term care, rehabilitation, or other clinical settings who are excited to transition into home health.

Responsibilities

  • Conduct comprehensive nursing assessments and develop individualized plans of care.
  • Manage a caseload of home health patients from admission through discharge.
  • Coordinate care with physicians, therapists, social workers, home health aides, and other members of the healthcare team.
  • Provide skilled nursing visits, education, and disease management.
  • Complete accurate and timely OASIS assessments and clinical documentation.
  • Educate patients and families to promote independence and confidence in managing their health.
  • Monitor patient progress and adjust care plans as needed.
  • Advocate for patients while ensuring the highest standards of quality, safety, and compassion.
  • Participate in case conferences and collaborate with the interdisciplinary team to achieve the best possible outcomes.
