About The Position

FlexStaff is hiring a part-time temporary Nurse Case Manager to coordinate the delivery of health care services for persons receiving services in an assigned residence and/or program unit. The Nurse Case Manager makes nursing assessments and diagnoses and works with other members of the Interdisciplinary Team (IDT) to achieve positive health outcomes in compliance with OPWDD and applicable regulations.

The schedule varies each week and consists of three days per week, coordinated between the site and employee. Hours are 9 a.m. to 5 p.m. The assignment is 13 weeks in Fishkill, New York, at $80.00 per hour.

Requirements

• Maintain a current New York State Registered Nurse license.
• Graduate from an accredited nursing program.
• Have excellent oral and written communication skills.
• Have computer skills; Microsoft Office competence is specified.
• Be able to travel to and access multiple agency locations.
• Have a flexible work schedule, with hours and/or days approved by the Director of Nursing.

Nice To Haves

• Bachelor’s degree preferred.
• Prior nursing experience and/or experience working with persons with developmental disabilities preferred.
• Electronic medical record experience preferred.

Responsibilities

• Coordinate the delivery of health care services for persons receiving services in an assigned residence and/or program unit.
• Make nursing assessments and diagnoses.
• Work with members of the Interdisciplinary Team to achieve positive health outcomes in compliance with OPWDD and applicable regulations.
• Perform job responsibilities in compliance with applicable laws, rules, regulations, policies, and procedures.
• Adhere to the agency’s corporate compliance program, including its code of business conduct, the NYS and federal False Claims Act, whistleblower protection, and mandatory reporting.
• Complete corporate compliance training at least annually.
• Cooperate fully in corporate compliance investigations and reviews.
