About The Position

The Registered Nurse serves as the case manager for assigned patients and as the teacher for patient’s caregiver(s). The RN case manager utilizes the nursing process and the six phases of the Alive CARE visit to provide safe, end of life care to the to the terminally ill patient/family unit. The primary responsibility of the RN case manager is to coordinate and provide hospice care for home care patients through the development, initiation and updating of a plan of care in collaboration with patient and family RN case managers provide direct patient care, oversee the interdisciplinary team and teach patients and families regarding the disease process, symptom management and offer support and strength during difficult times. Care delivery takes place in the patient’s home or in any other facility where the patient may reside or receive care. The RN case manager will be accountable for meeting Alive Care Standards and participating in an on-call rotation.

Requirements

  • Registered Nurse with a current nursing license.
  • Minimum one-year nursing practice or equivalent experience required.
  • Current Tennessee RN License or compact license.
  • If required to drive to carry out the duties of this position: current driver's license and automobile insurance as required by Tennessee State Law.

Nice To Haves

  • BSN preferred.
  • Knowledge of or willingness to learn hospice care.
  • Hospice, Oncology, or Home Health and/or long-term care preferred.
  • Hospice certification desirable.

Responsibilities

  • Accountable for meeting Alive Care Standards and participating in an on-call rotation.
  • Presents a Professional Image, representing Alive in a positive manner.
  • Provides Excellent Customer Service during visits.
  • Assesses, Plans, Intervenes and Educates patients and families.
  • Documents at the point of care, completing documentation of patient visit by verifying visit frequency and documenting accurate reflection of patient's condition during the patient visit; updates plan of care as needed.
  • Creates a lasting impression by providing encouragement and ensuring necessary supplies/medications are available.
  • Completes post visit follow-up according to Alive CARE visit standard.
